• Copy of a governmental picture identification of person filling out the request
• A self-addressed stamped envelope
• Check payable to "City of Central Falls"
• Cost: Walk-in copies $22.00 for the first certificate and $18.00 for each additional certificate when ordered simultaneously
• Mail-in Copies cost $25.00 and $18.00 for each additional certificate when ordered simultaneously
• Mail request to City Clerk's Office; 580 Broad St, Central Falls, RI 02863
• Final recording for the day is at 3:30 pm, including marriage licenses.
